The use of gamma knife radiosurgery has increased for treatment of brain tumors and vascular malformations over the past decades. Radiation therapy is originally the adjuvant treatment of choice for recurrent or residual pituitary adenoma. A 50-year-old woman visited our clinic and underwent operation under the impression of pituitary apoplexy. The primary histological diagnosis was pituitary adenoma with hemorrhage. After 2 months, the remnant tumor was treated with gamma knife radiosurgery. She readmitted to hospital with visual symptoms 10 months later. And then second operation was done and histological finding revealed fibrosarcoma. After several months, third operation was done because of recurrence of fibrosarcoma. There were no reported cases of radiation-associated sarcoma after gamma knife radiosurgery. We reports a case of fibrosarcoma arising from the pituitary fossa associated gamma knife radiosurgery of remnant tumor with pituitary apoplexy.
